FYI PTHS sports practices can not begin before 3:30 so athletes can get help after school, if needed, or attend the study table in the library. The library is now open after school for study.

PHYSICALS: Physicals can be done at the PTHS School-Based Health Clinic. Call for an appointment at 360-385-9400. If you use the PTHS Student Health Clinic you will also need this Registration/Consent Form (Also bring your sports forms, insurance info and the Registration/Consent Form.)

Sports Physicals for high school athletes.

Available at the School Based Health Centers

in Port Townsend and Chimacum.

Additional dates are available in Quilcene.

 

Port Townsend High School

School Based Health Center located in the Gael Stuart Building:

August 15th, 17th, 22nd.

9:00 am to 3:00 pm

 

Chimacum High School

School Based Health Center located off Chimacum High School Commons:

August 16th, 18th, 23rd

9:00 am to 3:00 pm

 

Quilcene

Jefferson Co. Public Health Clinic

June 12, July 5,  July 19,  August 2

Noon to 4:00pm

 

Cost $45, we can bill insurance or accept payment.

COSTS: Sports participants must purchase an ASB Card ($30), pay an Athletic User Fee of $75, and a sports fee of $65 for the 1st sport, $60 for the second sport and $55 for the third sport played per year.
Chimacum HS participants in Cross Country, Swim & Dive and Wrestling must have a CHS ASB card and complete the PTHS paperwork, along with paying the PTHS fees.

NOTE: ALL Student Athletes and Activity Participants must maintain full-time enrollment in FIVE classes, during their entire sport/activity season, to be eligible.

PTHS charges admission for football, volleyball, basketball, wrestling and soccer.
