If you'd like me to write you a teacher recommendation for college (and you've already asked me in person), please access this form, save your own copy, and complete it.

Once you've complete the form, please give me any cover sheets WITH YOUR PART COMPLETED I need to fill out and attach this form and any resume you have.  I would also like an electronic copy if possible.

Please make sure to always waive your right to see the letter (this lets the college know that I will be completely honest).  Of course, I will furnish you a copy for your records, or to be framed on your wall.
