FYI PTHS sports practices can not begin before 3:30 so athletes can get help after school, if needed, or attend the study table in the library. The library is now open after school for study.

Port Townsend School District Athletic Department


The Port Townsend School District Athletic Department is now using FinalForms, an online forms system that will help us boost compliance and safety. FinalForms allows you to complete and sign athletic participation forms from your computer, tablet or mobile device. 

FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, saving you time if you have multiple children.


You may login any time to update information including emergency medical contacts and information. You will be required to sign your forms once per year and any time you make an update.

Follow the prompts to create your account, create your students and sign your forms. Click on the Parent Playbook (attached) for additional guidance. 

If you require any support during the process, scroll to the page bottom and click 'Use Support'. 

You can pay online at: e~Funds for Schools or at the ASB Office.


PTHS requires the following fees to participate in the school athletic program:

  • ASB Card - $30 (purchased with 1st sport only)
  • Athletic Department Fee - $75 (paid with 1st sport only)
  • Sport Fee - $65 for 1st sport, $60 for 2nd sport, $55 for 3rd sport

Total needed for 1st sport of the school year - $170

Total needed for 2nd sport of the school year - $60

Total needed for 3rd sport of the school year - $55

PHYSICALS: Physicals can be done at the PTHS School-Based Health Clinic. Call for an appointment at 360-385-9400. If you use the PTHS Student Health Clinic you will also need this Registration/Consent Form (Also bring your sports forms, insurance info and the Registration/Consent Form.)

Chimacum High School

Cost $45, we can bill insurance or accept payment.

COSTS: Sports participants must purchase an ASB Card ($30), pay an Athletic User Fee of $75, and a sports fee of $65 for the 1st sport, $60 for the second sport and $55 for the third sport played per year.

Chimacum HS participants in Cross Country, Swim & Dive and Wrestling must have a CHS ASB card and complete the PTHS paperwork, along with paying the PTHS fees.

PTHS charges admission for football, volleyball, basketball, wrestling and soccer.
