Sports update letter from January 22, 2021

Students and families will still need to complete registration on Final Forms located on the high school website here. A current physical (within the last 24 months) must be on file to begin practices. 


If you need a sports physical, please plan ahead. Our school clinic is open on Monday morning from 9 - Noon and Tuesday afternoons from Noon – 3 pm. To make an appointment, call 360-385-9400, option 3.

The Port Townsend School District Athletic Department is now using FinalForms, an online forms system that will help us boost compliance and safety. FinalForms allows you to complete and sign athletic participation forms from your computer, tablet or mobile device. 

FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, saving you time if you have multiple children.

You may login any time to update information including emergency medical contacts and information. You will be required to sign your forms once per year and any time you make an update.

PTHS requires the following fees to participate in the school athletic program:

  • ASB Card - $30 (purchased with 1st sport only)
  • Athletic Department Fee - $75 (paid with 1st sport only)
  • Sport Fee - $65 for 1st sport, $60 for 2nd sport, $55 for 3rd sport

Total needed for 1st sport of the school year - $170

Total needed for 2nd sport of the school year - $60

Total needed for 3rd sport of the school year - $55
