Physicals are available in the main clinic or at Port Townsend High School from 8:30-3:30. For an appointment, stop by the Health Clinic in the Gael Stuart Building or call/text 360-390-8560.
You may also fill out an "Appointment Request Form" located in the black wall box to the right of the stairwell near the library. Physicals must be completed prior to starting practice.
Bring the Registration / Consent Form (versión en español), the Sports Physical Form and your insurance info to your appointment.
Fee waivers are available for our low-income students who participate in athletics and activities!
